This Liability Waiver and Assumption of Risk Notice applies to all participants in Team Tendy Lax training programs and events. By participating in any Team Tendy Lax activities, you acknowledge that you have read, understood, and agree to the terms outlined in this document.

1. Acknowledgment of Risks

Participation in lacrosse training, including goalie-specific training, involves inherent risks of injury. These risks include, but are not limited to, sprains, strains, fractures, concussions, heat-related illnesses, contact injuries, and other bodily injuries that may result from physical activity, contact with equipment, other participants, or playing surfaces. While Team Tendy Lax takes reasonable precautions to provide a safe training environment, these risks cannot be entirely eliminated regardless of the care taken to avoid injuries.

2. Voluntary Assumption of Risk

By participating in Team Tendy Lax programs, participants voluntarily assume all risks associated with lacrosse training activities. Participants acknowledge that they are responsible for their own safety and well-being and agree to follow all instructions provided by Team Tendy Lax coaches and staff. Participants accept full responsibility for any injury, loss, or damage that may occur during participation.

3. Release and Waiver of Liability

In consideration of being permitted to participate in Team Tendy Lax programs, participants hereby release, waive, discharge, and hold harmless Team Tendy Lax, its owners, coaches, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in such activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.

4. Medical Disclaimer and Participant Responsibility

Participants affirm that they are in good physical condition and do not suffer from any medical condition that would prevent or limit participation in lacrosse training activities. All participants acknowledge the following:

  • It is the participant's responsibility to consult with a physician before participating in any physical activity

  • Team Tendy Lax does not provide medical advice, medical clearance, or health assessments

  • Participants must immediately inform coaching staff of any injury, illness, or medical concern during training

  • Participants are responsible for providing accurate medical and emergency contact information to Team Tendy Lax

5. Emergency Medical Treatment

By participating in Team Tendy Lax programs, participants authorize Team Tendy Lax staff to obtain emergency medical treatment if necessary. Reasonable efforts will be made to contact emergency contacts provided by the participant, but immediate medical care may be provided without prior consent if deemed necessary by medical professionals or Team Tendy Lax staff in emergency situations.

6. Insurance Responsibility

Team Tendy Lax does not provide medical or accident insurance for participants. All participants are responsible for ensuring they have adequate health insurance coverage for any injuries that may occur during participation in Team Tendy Lax programs and events.

7. Conduct and Safety Requirements

All participants are expected to:

  • Follow all safety instructions and guidelines provided by Team Tendy Lax staff

  • Use proper equipment and protective gear as directed

  • Conduct themselves in a safe and respectful manner at all times

  • Immediately report any unsafe conditions or concerns to coaching staff

8. Photo and Video Usage

Participants grant Team Tendy Lax permission to use photographs, video recordings, and other media taken during training sessions and events for promotional and marketing purposes, including on the website, social media, and other marketing materials. Participants who wish to opt out of photo and video usage must notify Team Tendy Lax in writing.

9. Parental Responsibility for Minors

For participants under 18 years of age, parents and legal guardians acknowledge that they have read and understood this waiver and agree to its terms on behalf of the minor participant. Parents and guardians understand that by allowing their child to participate, they are not only giving up their own rights to sue but also those of the minor participant. Parents and guardians are responsible for ensuring their child is physically and mentally prepared for participation and for providing accurate medical and contact information.

10. Governing Law and Severability

This waiver shall be governed by and construed in accordance with the laws of the State of Kansas. Any disputes arising from this waiver shall be resolved in the appropriate courts of Kansas. If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.

11. Acknowledgment

By participating in Team Tendy Lax training programs and events, participants confirm that they have read this Liability Waiver and Assumption of Risk Notice, fully understand its contents, and voluntarily agree to its terms and conditions.
